What Causes Smile Lines?

smile lines treatment

Smile lines have a way of building slowly while still seeming to appear overnight. They’re one of the most common concerns our patients bring to us at Spa Medical. Understanding how this type of fine line forms and what you can do about it is a key first step in managing the issue your way.

How Do Smile Lines Develop?

Smile lines, clinically called nasolabial folds, are the creases that run from the sides of your nose down to the corners of your mouth. A second type, called perioral lines, forms directly around the lips themselves. Both develop through a combination of factors that work against your skin.

The collagen and elastin decline

Collagen is the structural protein that acts like the latticework of the skin. Elastin, as it sounds, gives your skin the ability to snap back into place when under tension. Starting in your mid-20s, your body produces less of both, and by your 40s, the signs of this decline tend to show up more prominently. According to research published in the American Journal of Pathology, skin collagen content of 80+ year olds was 75% lower than that of 18 to 29 year olds.

Volume loss beneath the surface

Your face relies on a delicately balanced arrangement of fat pads to maintain its shape and contour. As you age, these fat pads thin out and migrate downward under the influence of gravity. The result is a loss of support around the cheeks and mid-face, which causes the skin above to fold and crease rather than lie smooth.

Repetitive facial movement

Every time you smile, laugh or talk, the muscles in your face contract and the skin folds along the same lines. Over decades, those temporary folds become etched in. The skin around your mouth is especially prone to this because it’s thinner and more mobile than skin elsewhere on the face.

Sun damage and environmental stressors

UV radiation from the sun increases the rate at which collagen and elastin are broken down. Chronic sun exposure, even when you don’t end up with visible burns, degrades the skin’s structural proteins over time, making lines appear earlier and deepen faster. Smoking can compound this effect because it constricts blood vessels, reduces oxygen delivery to skin cells and introduces free radical damage.

What’s the Best Way to Treat Smile Lines?

At Spa Medical, we take a personalized approach to smile line treatment because the right solution depends on the severity of the lines, your skin quality and what kind of result you’re looking for. Our team of licensed medical professionals has been helping Middle Georgia patients restore their natural features for over 21 years.

Dermal fillers

Hyaluronic acid-based dermal fillers are among the most effective treatments for nasolabial folds. Injected directly into or around the fold, they physically replace the volume that’s been lost and lift the surrounding tissue. Results are immediate and can last anywhere from 12 to 18 months, depending on the product used and the individual patient. We offer several filler options that can address these concerns. At your complimentary consult, our providers will help you determine which one is right for you.

Neurotoxins

While neurotoxins like Botox and Dysport are not the primary treatment for deep smile lines, they play a supporting role. Relaxing the muscles around the mouth can reduce the repetitive motion that worsens the folds over time, and strategically placed injections around the lips can soften perioral lines. Many patients achieve their best outcomes through a combination of filler and neurotoxin treatment.

Sculptra

For patients who notice volume loss across a wider area, Sculptra is an injectable biostimulator that works differently from traditional fillers. Instead of adding volume directly, Sculptra’s active ingredient, poly-L-lactic acid, stimulates your body to produce new collagen over time. Results develop gradually over two to three months and can last up to two years, making it an excellent option for patients who prefer a subtle, progressive improvement.

Morpheus8 RF Microneedling

When skin laxity and texture are to blame for smile lines, Morpheus8 addresses both issues at once. This advanced platform combines microneedling with radiofrequency energy to stimulate collagen production in the deeper layers of the dermis while tightening the skin from within. It’s particularly effective for patients in their 40s and 50s who want to improve overall skin quality alongside targeted volume treatments.

The combination approach

The most significant improvements we see come from combining treatments strategically. A typical plan might involve filler to address the immediate fold, neurotoxin to reduce muscle activity and Morpheus8 to improve skin quality. During your consultation, our team will assess your specific concerns and recommend a plan that makes sense for your anatomy and your goals.
